Veterans who are appropriate candidates for this program are those who can live in an independent, supportive setting, and who are in need of affordable supportive housing community. Singleton Community Services and Build Us HOPE is committed to the operation of affordable housing for individuals who are in need of affordable housing and those in need of supportive services. Permanent supported housing: individuals can stay in a Micro Home or Apartment indefinitely.

The following criteria and documentation will be used in the evaluation of the rental application for residency. At the time of completing the application all individuals must:

Type of Service Records for Reservists
Many Reservists are issued a DD Form 256, when they separate from the military. Veterans who served in the National Guard or Air National Guard may have a form NGB Form 22, or NGB Form 22-a (corrected version). Former National Guard members may also have other forms of proof of service, such as a discharge paperwork from their state. But the NGB Form 22 is the generally accepted form for proof of service.
How to Get Copies of Military Records
The National Archives maintains most, but not all, military records. These military records should be available through a military records request. This is the first place to look for military service records, unless the individual serve prior to WWI, or was recently separated from the military.
Additional Ways to Prove Military Service
The DD Form 214, 215, 256, NGB Form 22 and similar forms are official proof of service documents. These are the forms usually required for applying for veterans benefits through the Department of Veterans Affairs and other official benefits. For example, you cannot apply for the GI Bill, VA Loan Certificate of Eligibility, VA health care, veteran’s burial benefits, or other major military veterans benefits without one of these forms.
Here are a few common methods veterans can use to verify military service:
Require Documentation Homeless (If applicable)
- Must be a Veteran of a branch of the United States Armed Forces.
- Continually homeless (streets or shelter) for a year, or more, or has had at least four episodes of homelessness (streets or shelter) in the past 3 years, AND has a disabling condition (chronically homeless).
- Can be diagnosed with one or more of the following conditions: substance use disorder ,serious mental illness, developmental disability (as defined in section 102 of the Developmental Disabilities Assistance Bill of Rights Act of 2000 (42 USC 15002), posttraumatic stress disorder, or chronic physical illness or disability.
- Living in places not meant for human habitations, cars, parks, sidewalks, abandoned buildings, on the street.
- Living in an emergency shelter including shelters for victims of domestic violence.
- Living in transitional or supportive housing for homeless persons who are documented as originally coming from the streets or shelter.
- Is being discharged within a week from an institution where s/he has been a resident for more than 30 days and no residence has been identified and the person(s) lack resources and support networks needed to obtain housing.
- In need of affordable housing or supportive housing.
- In any of the places mentioned above but is spending less than 30 days in a hospital or other institution and documented as coming from the streets or a shelter to enter permanent housing after FY2005.
Qualifying Criteria:
The following criteria and documentation will be used in the evaluation of the rental application for residency. At the time of completing the application all individuals must:
- Be at least 18 years of age.
- Provide a valid government issued ID or fingerprint documentation.
- Provide a valid social security card or proof thereof.
- Provide resident alien ID card (if applicable).
- Provide gross income documentation (or rental subsidy documentation) sufficient to pay the monthly rent.
- Complete a resident release and consent form.
- Provide documentation of homelessness in the Maricopa County area for at least one year, if applicable.

Proof of Military Service:

- Date and place of entry into active duty
- Home address at time of entry
- Date and place of release from active duty
- Home address after separation
- Last duty assignment and rank
- Military job specialty (MOS)
- Military education
- Decorations, medals, badges, citations, and campaign awards
- Total creditable service
- Foreign Service credited
- Separation information: type of separation (ex: honorable), character of service, authority and reason for separation, separation and reenlistment eligibility codes).

- Certification letter(s) from a homeless service provider or outreach worker
- Certification letter(s) from any other health or human service provider

- A refundable deposit equal to one month’s rent, due prior to move-in. (Waiver options)
- One pet is allowed with prior management approval and with a refundable, pet deposit based on the weight of the animal due in advance of move-in date.
- Security deposit is due prior to move-in. The deposit shall be refundable within 30 days of move-out, provided there are no damages to the unit and rent is paid in full.
- One (1) overnight visitor is allowed per night, with management’s prior approval. Overnight visits are limited to no more than three visitors per month and no individual visitor may stay overnight more than three times in any given week, Monday to Sunday.

- There is a one (1) couple or one (1) person maximum occupancy per home.
- An applicant must provide proof of a monthly gross income sufficient to pay full rent.
- The only exception to this requirement is for applicants referred through a homeless or housing organization(s)/agency, with a rental subsidy provided.
- Applicants may have zero income at the time of application, if their rent is approved to be subsidized by other agencies for a specified period of time.
- Shared community garden
- Each Micro Home shall be powered by Solar and have Sustainability

- Subsidy, if available, will be subtracted from the rent, so that the income calculation is 30% of tenant portion of the rent.
- If receiving entitlements/pensions/rental subsidy, applicant must provide current, updated documentation.
- Summary of Household Income Data form required.
- Resident must agree and consent to the release of personal information regarding income.

Crimes against persons committed by the applicant:
- Capital murder, murder/manslaughter, kidnapping, child molestation, rape, and crimes of a sexual nature, or arson.
- A pattern (3 or more instances) of continuing conduct/acts, regardless of type of severity which may interfere with the health, safety, or right to peaceful enjoyment of the premises by others.
